A referral can be made by statutory, voluntary, and private sectors. Once a referral has been received, an initial assessment will be carried out by a dedicated staff member to determine the level of support that is required by a potential service user.

This comprehensive assessment includes a home visit to the service user’s current place of residence. The assessment will allow us to decide if the applicant’s needs can be met by GPAH Resource Centre in conjunction with their families, care managers, and other healthcare professionals.

It sounds like you’re describing a process related to social services or healthcare support. Referrals can come from various sources such as government agencies (statutory sector), non-profit organizations (voluntary sector), and private entities. The goal of these referrals is to identify individuals who may benefit from specific services or support.

Once a referral is received, an initial assessment is conducted by a designated staff member. This assessment aims to determine the level of support needed by the potential service user. This initial assessment often involves a variety of factors, including the individual’s health, living situation, and overall needs.

A key part of this assessment is conducting a home visit to the service user’s current residence. This visit provides an opportunity to gather firsthand information about the individual’s living conditions, environment, and any challenges they may be facing.

Based on the comprehensive assessment, the staff member and the organization can then determine if the GPAH (assuming this is the name of the organization) Resource Centre can effectively provide the required support. This decision is often made in consultation with the service user’s family, care managers, and other healthcare professionals involved in the individual’s care. The collaboration between various stakeholders ensures a holistic and well-rounded approach to meeting the service user’s needs.

Overall, this process aims to ensure that individuals in need receive appropriate and tailored support through a collaborative effort between different sectors and professionals.
